Nipple-Areola Complex Relies Solely on the Dermal Plexus

Abstract

This case report describes a patient that underwent inferior pedicle breast reduction 2 years after sternal reconstruction by means of a right pectoralis major turnover flap. The turnover flap resulted in disruption of the parenchymal branches to the right breast, leaving the nipple-areolar complex (NAC) relying solely on the dermal and subdermal plexuses. The patient developed right NAC necrosis postoperatively. This clinical example supports the contention that the dermal and subdermal plexuses play a minor role in supplying the NAC.
