Nipple-Sparing Mastectomy and Nipple Ischemia

Abstract

Nipple-sparing mastectomy has rapidly gained acceptance because of its perceived aesthetic benefits and patient satisfaction. The technical aspects of the procedure continue to evolve as the rate of nipple ischemia approaches 15% in most series. Blood supply to nipple-areolar complex (NAC) comes mainly from perforating branches off the internal mammary artery. Knowledge of their course is necessary to preserve the blood supply to the NAC. Identifications of predisposing factors are paramount to reducing these complications. These include morphological factors (large breasts, ptosis), periareolar incisions, radiation, tobacco smoking, and oncological resections.
