Abstract
Posterior hip and buttock pain related to extra-spinal compression of the sciatic nerve has been previously described in multiple clinical conditions. A recent cadaveric study has demonstrated a potential new site for extra-spinal compression of the sciatic nerve between the posterior aspect of the greater trochanter and the lateral ischium. Pathologic contact of the greater trochanter and ischium can occur as the hip moves from flexion to extension with abduction and external rotation. Certain functional positions and structural abnormalities of the hip may predispose to impingement between the greater trochanter and ischium. To date, there is no clinical data regarding the diagnosis or treatment of this entity. Potential diagnostic and treatment options may be inferred from data regarding other sites of extra-spinal sciatic nerve compression . Future studies are indicated to determine the prevalence, diagnosis, and treatment of greater trochanteric-ischial impingement.
