Postburn Perineum Obliteration: Elimination of Perineal, Inguinal, and Perianal Contractures with the Groin Flap

Abstract

Deep burns of the perineum result in perineal obliteration; the total perineal contracture is often combined with perianal and inguinal contractures, which restrict the normal motion and make personal hygiene difficult. Contracture is formed without a fold; use of the local flap technique is therefore excluded. Skin transplants in the perineal zone often shrink. Elimination of such a complex perineal contracture is a challenge for surgeons. Success can be achieved only by using pedicle or free flaps. We noticed that a groin flap has excellent tissue for simultaneous perineal, inguinal, and anal reconstruction. The recommended surgical technique and results are shown in this chapter.
