Abstract

Multiple system atrophy is a neurodegenerative disorder (one of the atypical parkinsonian syndromes) characterized by the presence of akinetic-rigid parkinsonism, cerebellar ataxia, autonomic failure, and pyramidal signs. Other features include REM sleep behavior disorder, respiratory stridor, disproportionate anterocollis, orofacial dystonia, and mood disorder. There are no diagnostic tests available. There are also no disease-modifying treatments. Treatment is symptomatic. This chapter will review clinical diagnosis and the roles of levodopa, botulinum toxin injections, nonmedical and medical approaches to urinary incontinence, constipation and orthostatic hypotension, as well as antidepressants and anxiolytics for symptomatic treatment.
