Frau K., 61 Jahre alt und von Beruf selbständige Kauffrau, wird wegen des Verdachts auf eine autoimmun bedingte ANCA-assoziierte Vaskulitis für 5 Wochen mit Methotrexat und Prednisolon therapiert. Eine leichte Mitralklappeninsuffizienz im Rahmen eines Mitralklappenprolapses ist bei ihr seit vielen Jahren bekannt. Die stationäre Aufnahme erfolgt jetzt wegen einer seit ca. einer Woche zunehmenden Dyspnoe, die zunächst nur unter Belastung auftrat und jetzt schon in Ruhe besteht. Anamnestisch lässt sich erfragen, dass die Patientin in der Vergangenheit mehrfach aufgrund einer chronisch rezidivierenden Bursitis olecrani links punktiert wurde.
