Aktuelle Therapie des Oesophaguskarzinoms

Zusammenfassung

Die Therapie von Plattenepithelkarzinomen der Speiseröhre ist seit etwa 20 Jahren in ständiger Bewegung [9, 30, 31, 36, 54]. Zwei Aspekte werden vor allem diskutiert: Die Radikalität des chirurgischen Eingriffes [39] und mögliche multimodale Therapiemodelle [40]. Die Idee, die Tumorresektion mit anderen Maßnahmen zu kombinieren, geht auf Nakayama zurück [24, 29, 58]. Er konnte in einer ersten Studie einen positiven Effekt bei der Kombination von Operation und Bestrahlung [47] feststellen. Begründung für ein derartiges Vorgehen ist heute wie damals die im Vergleich zu anderen Tumoren des Gastrointestinaltraktes ungünstige Prognose des Plattenepithelkarzinoms der Speiseröhre. Beide Probleme sollen basierend auf dem Krankengut der Chirurgischen Klinik der Universität Erlangen-Nürnberg diskutiert werden.
