Zusammenfassung
Seit der Einführung des Gesundheitsstrukturgesetzes (GSG) konnten Fallpauschalen in einigen Bereichen der Chirurgie mit gut definierbaren Fallgruppen und wenig bis mäßig invasiven Operationen ihre Realisierbarkeit nachweisen (8). Typische Beispiele sind die Behandlung der Varicosis und der Leistenhernie. Dem gegenüber stehen die Bereiche mit weniger homogenen Fallgruppen und mit Operationen von erheblicher Invasivität. Hier unterliegen die Behandlungskosten in Abhängigkeit vom Risikoprofil des individuellen Patienten erheblichen Schwankungen. Als Beispiel sei die Herzchirurgie genannt, wo die Anwendbarkeit von Fallpauschalen in ihrer jetzigen Form bereits wieder ernsthaft in Zweifel gezogen wird (9).
