Summary
A change in therapeutic approach reduced the rate of appendectomies. In 1985, 409 appendectomies were performed, in 1986, 180 and in 1987, 130. In 1987 all 12 patients had a perforated appendix before hospitalization. In the last half of 1987, 165 patients with right-lower-quadrant tenderness were prospectively registered: 113 were hospitalized for observation, 50 appendectomies were performed and two remained ambulant. Nearly 50 % of patients not undergoing an operation still had abdominal pain compared to nearly 20% of those who did. Appendectomy is not necessary in every patient having acute right-lower-quadrant tenderness; hospital observation did not lead to increased perforations; the use of a checklist diminished the normal rate of appendectomies.
Zusammenfassung
Durch Änderung unseres Therapiekonzeptes ging — bei etwa gleichbleibender Einweisungshäufigkeit — die Zahl der Appendektomien von 409 im Jahre 1985 auf 180 in 1986 und auf 130 in 1987 zurück. Die im Jahre 1987 unter 130 Appendektomien operierten 12 Perforationen waren alle bereits vor der stationären Aufnahme eingetreten. Im 2. Halbjahr 1987 wurden 165 Patienten mit Schmerzen im rechten Unterbauch prospektiv erfaßt, 113 wurden zur stationären Beobachtung aufgenommen, 50 wurden operiert (jeder Dritte), 2 blieben ambulant. Im follow-up hatten fast 50% der Nichtoperierten noch Beschwerden, von den Operierten fast 20%. Ergebnis: Appendizitis nicht die häufigste chirurgische Erkrankung, sondern häufigste Operation; stationäre Beobachtung führt nicht zu mehr Perforationen; Erfassung mittels Checkliste senkt Zahl unnötiger Laparatomien, Anteil der blanden Befunde bleibt dennoch hoch.
