Femurkopffrakturen

Summary

Fractures of the femoral head are symptoms of different lesions. They are classified in

• (1) combination with posterior dislocation (Pipkin I-IV),

• (2) with anterior dislocation, as

• (3) fracture with head impression, and as

• (4) multiple fragments — fractures.

The patients had the following fractures: 39 (Pipkin I: 4×, II: 7×, III: -, IV: 9×; with anterior dislocation: 2 ×; multiple fragment fracture: 2 ×; small comminuted zone: 2; and 13 × impression with acetabulum fracture. The results show that in this kind of joint fracture an anatomical reduction with stabilisation is essential. In this way functional aftertreatment is possible and better results can be achieved.

Zusammenfassung

Femurkopffrakturen sind Symptom verschiedener Verletzungen und lassen sich folgendermaßen klassifizieren:

• 1. Fraktur kombiniert mit hinterer Luxation (Pipkin I-IV),

• 2. mit vorderer Luxation,

• 3. Kopfimpressionsfraktur;

• 4. Mehrfragment-Trümmerfraktur.

Von 1975–1985 konnten 39 Patienten, darunter 28 Männer und 11 Frauen mit einem Durchschnittsalter von 34 Jahren, behandelt werden. Pipkin I Verletzung: 4 ×, II: 7 ×, III: -, IV: 9 ×. Vordere Luxation mit Kopfabscherfraktur 2 ×, Kopftrümmerfraktur 2 ×, Fraktur mit kleiner Trümmerzone: 2 ×, 13 × Impression bei Acetabulumfraktur ohne Luxation. Es zeigt sich, daß, wie auch bei anderen Gelenkfrakturen, eine anatomische Reposition und stabile Versorgung eine funktionelle Nachbehandlung ermöglichen, wodurch sich bessere Ergebnisse erzielen lassen.
