Summary
Endoscopy is a safe technique with a sensitivity and specificity superior to those of conventional diagnostic procedures, which it has consequently replaced. In doing so, it has also altered many diagnostic and therapeutic concepts. Moreover, endoscopy has positively influenced the previously unaffected course of certain diseases, e.g. upper G. I. bleeding. Endoscopy benefits both the patient and the physician. Although no complete cost-benefit analysis is available, preliminary reports have been favourable. These aspects indicate that endoscopy is highly valuable in a clinical setting. It must be pointed out, however, that endoscopy as a whole has not yet been fully analyzed in any single given clinical study.
Zusammenfassung
Die Endoskopie ist eine sichere Technologie. Hinsichtlich Sensitivität und Spezifität ist sie herkömmlichen Verfahren überlegen und hat sie teilweise ersetzt. Die Endoskopie veränderte diagnostische und therapeutische Konzepte und es war möglich, so einen positven Einfluß auf den natürlichen Verlauf einer Erkrankung zu nehmen, z.B. akute obere intestinale Blutung. Weiterhin beinhaltet die Endoskopie einen Vorteil, sowohl für den Patienten als auch für den behandelnden Arzt. Diese positiven Kriterien haben unter einer breiten Anwendung der Endoskopie nicht gelitten. Eine umfassende Kosten-Nutzen-Analyse liegt z. Z. nicht vor, aber Teilaspekte zeigen eine sinnvolle Relation auf. Anhand dieser Bewertungskriterien kann die Endoskopie ihre hohe klinische Wertigkeit nachweisen. Einschränkend muß festgestellt werden, daß die Endoskopie in ihrer Gesamtheit nicht in einer einzelnen klinischen Studie überprüft wurde.
