Motilitätsstörungen nach erheblicher Blutung in die Orbita

Zusammenfassung

Nach Contusio orbitae kann es neben allen anderen Traumen auch zu einer isolierten Blutung in die Orbita kommen. Während anfanglich meist eine Immobilität des Bulbus eintritt, kann nach 3–4 Wochen eine bleibende Motilitätsstörung im Bereich des Blutungsmaximums beobachtet werden, die zu Diplopie führt. Es wird über 4 Fälle berichtet, bei denen durch die operative Revision nach ca. 2 Jahren mit Entfernung des organisierten Fibrinmantels und Rücklagerung des in Kontraktur befindlichen Antagonisten sowohl normale Motilität wie auch normale Binokularfunktion erreicht werden konnte. Postoperativ ist ein orthoptisches Training erforderlich, welches wegen der normalen Korrespondenz eine gute Erfolgschance hat.

Summary

Following orbital contusion an isolated haemorrhage can occur. Initially, an immobility of the bulbus is usually seen, and after three to four weeks, in the area of maximum bleeding, a permanent disturbance of motility may occur. Four cases will be reported, in which, after approximately two years, surgical removal of scar tissue and recession of the contracted antagonist resulted in both normal motility and normal binocular function. In such cases postoperative orthoptic treatment seems to be necessary, having a good chance of success due to the presence of normal correspondence.
