Das Gros aller Analfisteln (> 90%) entsteht auf dem Boden kryptoglandulärer Analinfekte. Dabei ist der Abszess das akute, die Fistel das chronische Infektstadium. Die Ausbreitung des Infektes erfolgt je nach Eindringtiefe in den präformierten Spalträumen der kulissenartig strukturierten Analregion. Es dominieren intersphinktere (> 40%) und transsphinktere (35–40%) Manifestationen. Erst die Sanierung des kryptoglandulären Quellbereiches kann zu einer Ausheilung führen. Dann sind Heilungsquoten über 95% zu erwarten. Auch bei korrekter Indikation und Technik muß mit 5–10% leichter Kontinenzminderungen gerechnet werden. Schwere Inkontinenzen II.-III.° sollten nicht auftreten.
