Summary

Amputation is the most frustrating endpoint of medical treatment for the surgeon as well as for the patient. To give a better chance for rehabilitation of the patients, the care should not end after amputation. If an unfavourable stump is the result of the amputation, e.g. too short for myoelectric prothesis, inadequate soft tissue coverage, all modern plastic surgery treatment modalities should be evaluated for the patient. Stump lengthening by distraction or free vascularized bone transfer (iliac crest, fibula or harvested parts of the amputate) should be used especially in upper extremity amputation.

In lower limb amputations length is the crucial point especially in lower leg amputations and foot amputations. Historic procedures like the Borggreve technique for patients with upper leg amputations due to trauma or tumor should be either done as a free lower leg graft or as described originally based on the intact femoral vessels.

By combining all the different treatment modalities, short and inadequate covered stumps can be avoided.

Zusammenfassung

Amputation gelten in der Chirurgie häufig als Resignation und stellen oftmals die frustrierende ultima ratio einer langwierigen Behandlung dar. Für den Patienten nicht weniger belastend stellt sie den Endzustand einer Behandlung dar, mit der dieser physisch und psychisch umgehen muß. Trotzdem wird der Stumpfformung und der eventuellen Stumpkorrektur oftmals wenig Bedeutung geschenkt. Durch Ausschöpfung verschiedener plastisch chirurgischer Operationstechniken ist bei primärer Amputation häufig ein Funktionsgewinn möglich, Stumpfkorrekturen sollten die Erfordernisse des Patienten und der Prothesenhersteller berücksichtigen.
