Zusammenarbeit von Chirurgie und Pathologie im diagnostischen Qualitätsmanagement: Die tägliche Praxis — wo können Defizite auftreten?

Communication is the key to a well-functioning interaction of high quality between surgeon and pathologist. Contact points for communication exist during the pre-, intra-, und postoperative period: Preoperatively the pathologists’ evaluation of biopsies is the gold standard for the establishment of the diagnosis and selection of the therapy. Postoperatively the pathology report is the quality characteristic, particularly for resections in surgical oncology. A further contact point exists during the intraoperative period, when the pathologist is embedded in the on-the-spot evaluation of the specimen. Modern surgical concepts (e.g. prophylactic resections) require the sharing of responsibility, because the histopathologic diagnosis can be difficult and the clinical implications may be severe.

Zusammenfassung

Kommunikation ist der Schlüssel zur Qualität in der Interaktion zwischen Chirurgen und Pathologen. Schnittstellen hierfür gibt es prä-, intra-, und postoperativ: Präoperativ bedient der Pathologe mit der Beurteilung der Biopsien den Goldstandard in der Diagnosesicherung und Therapieentscheidung. Postoperativ ist der pathologische Befund das Qualitätsmerkmal, insbesondere für onkologisch-chirurgische Resektionen. Eine weitere Schnittstelle wird geschaffen, wenn ein Pathologe intraoperativ in die Befundung einbezogen wird, in Form eines „Präparatedienstes“ im OP-Saal. Moderne chirurgische Konzepte (z.B. prophylaktische Resektionen) erfordern eine Aufteilung der Verantwortung, weil die histopathologische Diagnose schwierig und die klinische Konsequenz weitreichend sein kann.
