Zusammenfassung
Die Patella stellt ein Sesambein im Quadrizepsmuskel dar und hat im Alltag verschiedene Funktionen. Neben Kraftübertragung und Hebelarm für den Quadrizeps beeinflusst sie gleichzeitig den Reibkoeffizienten zwischen Quadrizeps und Femur. Ihr Fehlen macht sich nicht zuletzt auch kosmetisch bemerkbar. Die ersten entwickelten Knieendoprothesen berücksichtigten die patellofemorale Gelenkfläche noch wenig. Im Rahmen der Nachuntersuchungen von eingebauten Knieendoprothesen zeigte sich, dass dem Peripatellarbereich mit durchschnittlich 10% der geklagten Patientenbeschwerden eine wichtige Rolle zukommt [1, 2, 5, 7, 16, 18, 20, 28, 34]. Diagnose, Operateur und Prothese haben einen Einfluss auf die Wahl für oder gegen einen Patellaersatz. Als Hauptprobleme im patellofemoralen Bereich gelten Schmerzen, Lateralisations- und Verkippungstendenzen der Patella und seltener auch Weichteilimpingement, Frakturen und Lockerungen der Patellaprothese.
