Zusammenfassung
Sowohl die Übertragbarkeit von Krankheiten als auch ökonomische Gesichtspunkte führen zu einer zunehmend engeren Indikationsstellung für die Transfusion von Blutkomponenten. Der Sauerstofftransport durch das Blut scheint beim nicht kardiovaskulär vorerkrankten Patienten auch bei einem Hämoglobinwert von 7–8 g/dl gewährleistet zu sein, bei entsprechender Vorerkrankung wird ein Hb-Wert von 10 g/dl empfohlen. Indikationen für eine Frischplasmagabe sind hauptsächlich Massivtransfusionen und komplexe Gerinnungsstörungen.
Die Grundlage zur Vermeidung allogener Blutübertragungen ist eine blutarme Operationstechnik. Für elektive Eingriffe stehen die ▸ Eigenblutspende und die Hämodilution, bei präoperativ anämischen Patienten evtl. unter begleitender Therapie mit Erythropoetin, zur Verfügung. Bei Notfalleingriffen und elektiven Eingriffen mit größerem Blutverlust sollte die Rückgewinnung des Wundblutes durch ▸ Autotransfusion intra- und postoperativ genutzt werden. Kontrollierte Hypotension, bestimmte Anästhesieverfahren und Antifibrinolytika können intraoperative Blutverluste verringern.
Bei der Anwendung von Blutprodukten ist der Dokumentation ein besonderes Augenmerk zu widmen.
