Zusammenfassung

Bei einer Vielzahl von malignen Tumoren wird die Strahlentherapie unabhängig vom Alter entweder mit kurativer oder mit palliativer Zielsetzung eingesetzt. In multimodalen Therapieprotokollen richtet sich der Stellenwert der einzelnen Therapiekomponenten Chirurgie, Strahlentherapie und pharmakologische Therapie (Chemotherapie, Immuntherapie etc.) vorrangig nach der lokalen oder systemischen Tumorausbreitung. Sowohl die Therapiestrategien als auch ihre einzelnen Komponenten wurden und werden üblicherweise für jüngere Patienten entwickelt und bemessen. Im Gegensatz dazu beeinflussen beim älteren Patienten häufig vorliegende medizinische Begleiterkrankungen maßgeblich die durchzuführende Therapie. Mehr noch als die moderne Chirurgie stellt die Radiotherapie eine funktions- und organerhaltende Therapiemaßnahme mit kurativem Potenzial bei lokalisierten Tumorerkrankungen dar, die infolge geringer Morbidiät und so gut wie fehlender therapiebedinger Mortaliät im Alter besonders indiziert ist. Daher wird die Indikation zur Radiotherapie relativ selten durch hohes Alter und die sich daraus ergebenden Begleitumstände relativiert oder aufgehoben.
