Zusammenfassung
Die Ätiologie der entzündlichen Wirbelsäulenerkrankungen ist in vielen Punkten noch unklar. Auch aufgrund klinischer Aspekte ist die Zuordnung oft problematisch. Im Wesentlichen wird die infektiöse („bakterielle“, „pyogene“) Spondylitis von den Spondylarthropathien, v. a. aus dem rheumatischen Formenkreis, bei denen kein Erregernachweis gelingt, unterschieden. Bei den nicht-infektiösen Spondylarthropathien wird eine genetische Disposition durch das Vorhandensein bestimmter Gewebeverträglichkeitsantigene („HLA-Antigene“) angenommen. Vermutlich wird die Erkrankung durch eine Infektion ausgelöst (= „Starterwirkung“ der Infektion bei entsprechender genetischer Disposition). Die Tabelle 1 listet die wichtigsten Formen der entzündlichen Wirbelsäulenerkrankungen übersichtlich auf.
