Zusammenfassung
In dem Bemühen, bei Ernährungsproblemen eine längerfristige parenterale Ernährung zu verhindern oder zu beenden, wurden transnasale Sondentechniken [31] und geeignete Sondennahrungen (bilanzierte Formeldiäten, Trink-und Sondennahrung) entwickelt. Nachteile nasoenteraler Sonden (Irritationen im Nasen-Rachenraum, Dislokation, kosmetische Beeinträchtigung) führten zur perkutanen endoskopisch kontrollierten Gastrostomie [2,4,9, 10, 15, 20– 22, 27, 29, 30, 35, 38–40,42). Sie ermöglicht — ohne chirurgischen Eingriff— eine schnelle, einfache, sichere und komplikationsarme transorale oder transkutane Applikation des Katheters in den Magen (PEG) oder das Duodenum und somit eine langfristige physiologische enterale Ernährung jener Patienten, bei denen eine orale Nahrungszufuhr nicht oder noch nicht möglich ist [36]. Dadurch wird die Anlage einer Witzel-Fistel überflüssig.
