Zusammenfassung
Unmittelbar bei Eintreffen des Notarztes am Unfallort hat dieser neben medizinischen Aufgaben zunächst organisatorische Aufgaben zu übernehmen. Vorrangig zu klären sind Fragen nach der Anzahl der Verletzten und der Schwere der Verletzungen. Hiernach ist zu prüfen, ob die personellen wie technischen Ressourcen ausreichen oder aber noch weitere Rettungsmittel oder z. B. Bergungsgerät angefordert werden muß. Diese Aufgaben sind überwiegend zu delegieren an Mitarbeiter der Rettungsorganisationen bzw. Feuerwehr. Das Absichern der Unfallstelle als auch das Retten der Patienten aus Autowracks muß mit dem Einsatzleiter der Feuerwehr und der Polizei eng abgestimmt werden, damit es nicht zur Gefährdung des Patienten und des rettenden Personals kommt. Nach Sichtung aller Patienten muß entschieden werden, ob der Notarzt vor Ort allein mit der Situation zurecht kommt, evtl. unter Delegation der Therapie Leichtverletzter an das Rettungspersonal, oder ob weitere Notärzte erforderlich sind (Abb. 8.1). Der zuerst am Einsatzort eintreffende Notarzt ist automatisch ärztlicher Einsatzleiter an der Unfallstelle und für die Einweisung der nachkommenden Rettungsmittel zuständig. In Städten und Regionen, wo der Leitende Notarzt als Institution eingerichtet ist, wird dieser von der Rettungsleitstelle bei Großunfällen bestellt und übernimmt dann bei Eintreffen am Unfallort die weitere Koordination, nachdem er sich von dem zuerst am Unfallort eingetroffenen Notarzt in die Situation hat einweisen lassen.
