Besonderheiten der Divertikulitis beim Diabetiker

Zusammenfassung

Besonders in der Literatur der sechziger- und siebziger Jahre finden sich immer wieder Hinweise auf eine klinische Assoziation zwischen der Divertikulitis und dem Diabetes mellitus. Dabei wird aufgrund von einigen Fallstudien und retrospektiven Studien auf einen akzelerierten Verlauf der Divertikelerkrankung und eine besonders schlechte Prognose der Divertikulitis bei Diabetes aufmerksam gemacht. Andererseits schien sich im Hinblick auf die Hypothese von der faserarmen Ernährung als einer gemeinsamen Ursache von Divertikelerkrankung und Diabetes sowie anderer westlicher Zivilisationskrankheiten ein ätiologischer Zusammenhang zwischen der Divertikulitis und dem Diabetes zu ergeben. Für diese populäre These und die Möglichkeit der Prävention und Therapie von Divertikulitis und Diabetes durch faserreiche Ernährung liegen nach den Standards der „Evidence-based Medicine“ keine Beweise aus klinischer Forschung vor.

Im Gegensatz zu etwaigen plausiblen Hinweisen aus Fallstudien und klinischen Erfahrungen ergab eine systematische Literaturanalyse weder einen schlüssigen Beweis für eine Syntropie von Divertikelerkrankung und Diabetes noch einen Anhalt dafür, dass der Diabetes als ein Risikomarker oder Faktor für einen prognostisch ungünstigen Verlauf der Divertikelerkrankung eingestuft werden kann.
