Multimodale Therapie beim sakralen Rezidiv des Rektumkarzinoms

Summary

Curative treatment options for sacral recurrence of rectal carcinoma are very limited and reduced to a multimodal combination of chemotherapy, radiation (hyperthermia) and surgery. The primary treatment goal, however, must be the Ro resection of the sacral recurrence. A retrospective analysis of 143 of our own patients with sacral recurrences demonstrates that the resectability of the recurrent tumor has the greatest significant influence on long-term survival. The possibility of a curative reoperation depends highly on the pattern of organ involvement. Local luminal recurrences at the site of the anastomosis may be treated like primary tumors. In well-selected exceptional cases, pelvine exenteration may be indicated in young patients with no multimodal therapy options left. In this context, a new prognosisoriented classification for locoregional recurrences is introduced, possibly improving indication strategies for sacral recurrences

Zusammenfassung

Beim sakralen Rezidiv des Rektumkarzinoms ist die Möglichkeit einer kurativen Therapie meist auf eine multimodale Kombination von Chemotherapie, Strahlentherapie (Hyperthermie) und Chirurgie reduziert, muss jedoch mit dem Behandlungsziel der Ro-Resektion des sakralen Rezidivtumors verbunden sein. Die retrospektive Analyse des eigenen Patientenguts von insgesamt 143 sakralen Rezidivpatienten zeigt, dass die Resektabilität des Rezidivtumors den wichtigsten Einflussfaktor für die Langzeitprognose darstellt. Die Möglichkeit einer kurativen Rezidivoperation ist im Wesentlichen vom Muster des Organbefalls abhängig. Anastomosenrezidive sind wie Primärtumoren zu behandeln. Im gut selektierten Ausnahmefall kann beim jungen, multimodal austherapierten Patienten die pelvine Exenteration angezeigt sein. In diesem Zusammenhang wird eine neue prognoseorientierte Rezidivklassifikation vorgestellt, die zur Verbesserung der Indikationsstrategie beitragen kann.
