Zusammenfassung
Leitsymptome von Motilitätsstörungen des tubulären Ösophagus sind Dysphagie und Thoraxschmerzen. Beseitigung bzw. Linderung dieser beiden Symptome ist Ziel jeder Therapie. Die Indikation zur Therapieeinleitung orientiert sich zum einen an den Ergebnissen der ösophagealen Funktionsdiagnostik, v. a. der Manometrie, und zum anderen an der Häufigkeit und Schwere der Symptome. Immer wieder überraschend ist die Diskrepanz zwischen manometrisch faßbarer ösophagealer Funktionsstörung und Schweregrad der Symptome. Diese Diskrepanz weist auf unser immer noch ungenügendes Wissen über die Pathophysiologie hin. Vor allem das Zusammenspiel zwischen Sensibilität und Motorik der Speiseröhre ist erst in Ansätzen erforscht. Voraussetzung für die Einleitung einer Therapie von vermuteten oder objektivierten Motilitätsstörungen des tubulären Ösophagus ist in jedem Falle der Ausschluß einer organischen Erkrankung der Speiseröhre mittels bildgebender Verfahren. Die Endoskopie ist zwar sensitiver bei der Erkennung von Schleimhautdefekten, andererseits erlaubt die Röntgenuntersuchung mit Kontrastmittel auch die Beurteilung von Bewegungsabläufen. Somit ergänzen sich beide Untersuchungsmethoden in ihren Aussagemöglichkeiten.
