Zusammenfassung

Der Tuberkel ist histologisch das charakteristische, aber keinesfalls spezifische Substrat der Tuberkulose. Maßgebend für seine Bildung ist die allergische Reaktion des Wirtes auf das Mycobacterium als Antigen (Humphrey u. White, 1970). Morphologisch lassen sich zwei wesentliche Entwicklungsphasen des tuberkulösen Knötchens und zwar eine prägranulomatöse und eine granulomatöse Phase unterscheiden. Die erste Phase ist durch eine unspezifische Entzündung mit zahlreichen immigrierten Leukocyten und säurefesten Stäbchen gekennzeichnet. Daran schließt sich etwa 3 Wochen nach dem Infekt die zweite, granulomatöse Phase an. Sie ist ein morphologischer Ausdruck der eingetretenen Sensibilisierung (Tuberkulintest wird positiv). Es entwickeln sich epitheloidzellige Tuberkel mit Riesenzellen, vornehmlich vom Typ Langhans, und einem lymphocytären Saum um das gebildete Granulom. Im Zentrum des Granuloms kann es zur Verkäsung (eine durch hohen Lipidgehalt geprägte Koagulationsnekrose) kommen, die keinesfalls eine obligate Erscheinung der Hauttuberkulose ist.
