Zusammenfassung

Die Einschätzung und prognostische Beurteilung einer Rhythmusanomalie hängen von einer Reihe verschiedener Faktoren ab. Sie können im individuellen Fall erst nach einer ausführlichen Diagnostik und unter Berücksichtigung der Gesamtlage erfolgen. Grundsätzlich sollten folgende Fragen geklärt werden (Tabelle 7.1):

  1. a)

    Welcher Art ist die vorliegende Arrhythmie, was sind ihre Ursachen und Entstehungsbedingungen?

  2. b)

    Unter welcher Erscheinungsform manifestiert sich die Rhythmusstörung, wie wirkt sie sich auf Herz und Kreislauf aus?

  3. c)

    Ist ein organisches Herzleiden faßbar, wie sind dessen Akuität, Dynamik und Progredienz (z. B. umschriebene Narbe nach Myokarditis oder fortschreitende koronare Herzkrankheit)?

  4. d)

    Wie ist der funktionelle Zustand des Herzens, liegt eine latente oder manifeste Herzinsuffizienz vor, besteht eine kausale Verknüpfung mit der Arrhythmie?

  5. e)

    Wie lange besteht die Arrhythmie bereits, hat die Rhythmusstörung bisher geschadet, schadet sie jetzt, wird sie dies in Zukunft tun?

  6. f)

    Läßt sich die Arrhythmie kausal oder symptomatisch wirksam behandeln?
