Hämolytische Syndrome in der Geburtshilfe: Therapeutische Möglichkeiten vor dem Hintergrund einer gestörten Prostacyclinsynthese

Zusammenfassung

Lebensbedrohende Erkrankungen während der Schwangerschaft sind häufig durch ein akut erworbenes hämolytisches Syndrom kompliziert. Das Syndrom ist charakterisiert durch eine ausgeprägte Anämie, das Auftreten von Fragmentozyten und Retikulozyten im peripheren Blut, eine Hämoglobinämie und in schweren Fällen durch eine Hämoglobinurie. Bilirubin und Laktatdehydrogenase werden im Serum erhöht gefunden, der Coombs-Test und Teste auf antirethrozytäre Antikörper sind negativ und schließen immunhämolytische Anämien aus. Die Leukozytose ist inkonstant, und eine Thrombozytopenie kann nahezu regelmäßig beobachtet werden. Klinisch findet sich in vielen Fällen ein Nierenversagen (11).
