Zusammenfassung
Die zentrale pontine Myelinolyse stellt eine v.a. im Hirnstamm lokalisierte akute Entmarkungskrankheit dar, die geeignet ist, akut, subakut oder auch erst nach längerem Verlauf den Tod herbeizuführen. Bei unklaren AuffindungsVerhältnissen läßt sich vielfach auch klinisch die Diagnose nicht stellen und es entstehen Abgrenzungsschwierigkeiten gegenüber Vergiftungen. Die Kenntnis dieses Krankheitsbildes hilft vor dem Einsatz umfangreicher und aufwendiger chemisch-toxikologischer Verfahren, frühzeitig die Diagnose zu stellen. Lokalisation und typische morphologische Merkmale werden beschrieben.
Summary
Central pontine myelinolysis is a demyelinating disease found mainly in the brain stem that causes death after an acute or subacute course, or not until after some months or years. If an unconscious person is found in inexplicable circumstances, correct diagnosis by clinical investigation is very difficult, especially since differentiation from poisoning is not always easy. Knowledge of this disease aids early confirmation of the diagnosis and avoids a lot of expensive and time-intensive toxicological examinations. Localization, typical morphology, and history are described in five cases.
