Zusammenfassung

Die Indikation zur Operation bei einer Colitis ulcerosa muß vor dem Hintergrund gesehen werden, daß die leichten und mittelschweren Verlaufsformen durch medikamentös-konservative Maßnahmen effektiv behandelt werden können. Gleichzeitig kann festgestellt werden, daß die Colitis ulcerosa im Gegensatz zum M. Crohn eine chirurgisch heilbare Erkrankung darstellt. Die Indikation zur operativen Therapie bei der Colitis ulcerosa kann für 3 Situationen festgelegt werden [8]:

  1. 1)

    Akute Notfallsituation bei Vorliegen einer Perforation, Peritonitis oder akuten Blutung;

  2. 2)

    Versagen konsequent durchgeführter konservativer Therapiemaßnahmen mit Auftreten lokaler Komplikationen und Herabsetzung der individuellen Lebensqualität;

  3. 3)

    erhöhtes Malignisierungsrisiko.
