Ektopes ACTH-Syndrom - Diagnostische und therapeutische Aspekte

Zusammenfassung

Bereits 1952 beschrieb Thorne das gemeinsame Vorkommen eines Bronchialkarzinoms mit einem Cushing-Syndrom [21]. 1961 konnte Christy ACTH-Aktivität im Plasma von Patienten mit Bronchialkarzinomen nachweisen [3]. Meador und Mitarbeiter [14] wiesen 1962 bei 5 Patienten als Ursache ihres Cushing-Syndroms extrahypophysäre Tumoren nach, 1965 wurde von Liddle der Begriff „ektopes ACTH- Syndrom“geprägt [13]. Dieses Syndrom gehört sicherlich zu den am besten untersuchten paraneoplastischen Syndromen (Übersicht bei [8,19]). Diese Tumoren gehören zu den sog. APUDomen. Am häufigsten wurde eine ektope ACTH-Produktion für das kleinzellige Bronchialkarzinom nachgewiesen [7, 8], eine paraneoplastische ACTH-Sekretion wurde aber auch bei Pankreastumoren, Thymomen, Karzinoiden, medullären Schilddrüsenkarzinomen und Phäochromozytomen nachgewiesen [11].
