Zusammenfassung
Trimazosin [2-hydroxy-2 methylpropyl 4-(4 amino 6, 7, 8-trimethoxy-2-2 quinazolinyl) 1-piperazinecarboxylat monohydrochloride monohydrat] (Abb. 1) hat sich in tierexperimentellen [6, 7, 8,12,13,14] und humanpharmakologischen Untersuchungen [1, 2, 3, 9, 10, 15, 17, 18] als potenter Vasodilatator mit vornehmlicher Wirkung auf die Widerstandsgefäße gezeigt. Die gefäßrelaxierende Wirkung auf die kleinen Arteriolen und Venolen beruht auf der Blockade der α-1-Rezeptoren und Hemmung der Phosphodiesterase [3, 8]. Bei hypertensiven Patienten führt Trimazosin nach p.o. Gabe zu einer effektiven und dosisabhängigen Senkung des systolischen und diastolischen Blutdrucks und zur Abnahme des peripheren Gefäßwiderstandes ohne wesentliche Änderung der Herzfrequenz [2, 9, 17]. Ebenso kommt es bei Patienten mit Herzinsuffizienz zu einer Verbesserung der Hämodynamik mit HZV-Zunahme, zum Rückgang erhöhter Druckwerte im kleinen Kreislauf, zu einer Senkung des peripheren und pulmonalen Gefäßwiderstandes sowie zu einer geringen Abnahme der Herzfrequenz [1, 3, 10, 15, 18]. Mittels Venenverschlußplethysmographie konnte eine Wirkung auf die Kapazitätsgefäße wahrscheinlich gemacht werden [10]. Eine rasche Resorption (Cmax nach 1 bis 2h), gute Bioverfügbarkeit, eine Halbwert-zeit der β-Phase von 3–6 h und gute Verträglichkeit sind weitere Eigenschaften.
