Summary
The clinical picture of arteriovenous fistula (avf) is represented with respect to etiology, pathophysiology, diagnostic measures, and therapy. After providing a historical review of the developments of the surgical therapy the case history of a patient is presented who suffered from severe cardio-vascular decompensation due to a traumatic avf of the iliac vessels which was acquired 42 years ago. Additionally, the severely elongated and dilated afferent artery showed a large aneurysm. The surgical procedure of resection of avf and reconstruction of arterial and venous continuity is described. The principles and indications of various established surgical procedures are discussed.
Zusammenfassung
Das Krankheitsbild der arteriovenösen Fistel (avF) wird hinsichtlich Ätiologie, Pathophysiologie, Diagnostik und Therapie dargestellt. Nach einem historischen Überblick über die Fortschritte der chirurgischen Therapie wird die Kasuistik eines Patienten mit einer 42 Jahre lang bestehenden traumatisch erworbenen avF der Beckengefäße mit extremer kardiovaskulärer Dekompensation vorgestellt. Es bestand zusätzlich ein Aneurysma der stark elongierten und dilatierten afferenten A. iliaca. Die chirurgische Therapie durch Resektion der avF und Rekonstruktion von arterieller und venöser Strombahn wird beschrieben. Die Prinzipien und Indikationen verschiedener etablierter Operationsverfahren werden erläutert.
