Zusammenfassung
Kältechirurgie ist eine ausgezeichnete, wohl bekannte Methode, Tumorgewebe durch lokale Gefriervorgänge zu zerstören. Die ersten Untersuchungen über die zu erwartende Immunantwort nach Kältechirurgie wurden von R. J. Ablin und B. Helpap beschrieben.
Bei 32 Patienten mit einem malignen Melanom im Stadium II (pN1, pM1) führte die zeitlich versetzte, wiederholte kältechirurgische Zerstörung von Metastasen zu einer Reduktion auch peripherer Tumormassen.
In dem die Therapie begleitenden immunologischen Monitoring im peripheren Blut konnte der in der Literatur beschriebene booster-Effekt ebenfalls nachgewiesen werden.
Um das immunologische Geschehen weiter zu untersuchen, entwickelten wir eine randomisierte klinische Studie, in der das periphere Blut immunologisch untersucht wurde. Anhand der zur Zeit in der Studie rekrutierten 16 Patienten werden die ersten Ergebnisse diskutiert.
