Abstract
The first step in laparoscopy is pneumoperitoneum, the technique for which is well known: a needle is inserted in the abdominal cavity and gas is introduced through it. The abdominal cavity, which is normally virtual, is thus transformed into a real cavity that can be entered by the trocar and then by the laparoscope. This initial step is very important because only if it is successful can good laparoscopic results be obtained. It should be pointed out here that from a technical viewpoint it is difficult to perform pneumoperitoneum, and unsuccessful or unsatisfactory results depend in most cases upon particular conditions, above all the presence of extensive adhesions. Sometimes, however, even the most skilled operator can encounter difficulty in apparently simple cases.
