Transösophageale Endosonographie (TEE) bei Herz- und Aortenerkrankungen

Summary

The transesophageal echocardiography has provided a new acoustic window to the heart, the aorta and the mediastinum. The ultrasound probe will be used monoplane, biplane and multiplane in various image planes with pulsed, continuous and color-coded Doppler. The clinical indications for this procedure have included thoracic aortic dissection, prosthetic cardiac valve dysfunction, detection of an intracardiac source of embolism, endocarditis, cardiac and paracardiac masses, and mitral regurgitation. It is also proved to be useful in assessment of critically ill patients in whom standard transthoracic echocardiographic images did not provide complete assessment. The TEE is not only opening a new window to the chest, but it is also extending the diagnostic spectrum.

Zusammenfassung

Die transösophageale Echokardiographie (TEE) bietet die Möglichkeit der Beurteilung des Herzens, der thorakalen Aorta und des Mediastinums. Eine monoplane, biplane oder multiplane Sonde ermöglicht die Beschallung in verschiedenen Schnittebenen mit der gepulsten, kontinuierlichen und Farbdoppleroption. Die Komplikationsrate liegt unter 1%. Gesicherte Indikationen sind die Aortendissektion, die Klappenprothesendysfunktion, die Suche nach intrakardialen Thromben bei Embolie, die Klappenendokarditis, ein unklarer Schockzustand, Infarktkomplikationen, postoperative Komplikationen und transthorakal eingeschränkte Untersuchung bei wichtiger Fragestellung. Die transösophageale Echokardiographie eröffnet ein neues Fenster in den Thoraxraum und erweitert das diagnostische Spektrum.
