Zusammenfassung

Jedem primär erfolgreichen lumeneröffnenden peripheren Gefäßeingriff, sei dieser kathetertechnisch oder chirurgisch, liegt die immanente Gefahr eines Reverschlusses zugrunde. Schon 1973 kam Szylagyi zu dieser Erkenntnis, als er feststellen mußte, daß sich in über 30% der femoropoplitealen Venen-Bypasses fibröse Stenosen entwickeln. Er folgerte aus dieser Erkenntnis, daß diese Stenosen für eine große Anzahl Reverschlüsse veranwortlich sein könnten, und befürwortete ein aggressives, prophylaktisch chirurgisches Vorgehen [1]. Zahlreiche Arbeiten haben seither die Notwendigkeit von regelmäßigen Nachkontrollen nach peripherer Gefäßchirurgie und nach perkutaner transluminaler Angioplastie (PTA) belegt [2–6]. Diesen verdanken wir wesentliche Erkenntnisse über das physiologische Verhalten der Gefäßwand und deren Interaktionen mit den zirkulierenden Blutbestandteilen [7, 8]; sie führten zum Verständnis für die Thrombusbildung auf der Basis endothelial vorgeschädigter Gefäßwandabschnitte wie auch für die Entstehung der gefürchteten myointimalen Hyperplasie [9–13].
