Summary

Local excision of small mobile carcinomas of the lower rectum is performed by electrocautery; exposition of the tumour requires use of a self-retaining retractor. The experienced surgeon can remove more proximal tumours by microsurgery using a rectoscope and CO 2 insufflation (according to Buess). Our results from 67 patients clearly demonstrate that local excision for cure is only justified in cases of T1 “low risk” tumours.

Zusammenfassung

Die lokale Exzision von kleinen beweglichen Rektumkarzinomen im distalen Rektum erfolgt elektrochirurgisch nach Einsetzen von Sperrern. Höher gelegene Tumoren können bei entsprechender Erfahrung über ein Operationsrektoskop unter CO2–Insufflation (nach Buess) mikrochirurgisch abgetragen werden. Unsere eigenen Erfahrungen bei 67 Patienten zeigen deutlich, daß eine lokale Exzision aus kurativer Sicht nur im Stadium T1 „low risk” gerechtfertigt ist.
