Zusammenfassung
Jede Gewebsverletzung führt zu einer Stimulation der endothelialen Prostanoidproduktion [4]. Neben einer erhöhten lokalen Synthese der Prostaglandine am Ort der Verletzung findet man auch einen Anstieg der Konzentration im peripheren Blut. So wurden erhöhte Plasmaspiegel von Prostacyclin (PGI2), Thromboxan (TxA2), Prostaglandin (PG)F2α , PGM und PGE2 bei Patienten mit Polytrauma, Knochen-frakturen oder Thoraxverletzungen gemessen [2, 3, 4]. Beim Bruch eines Knochens ensteht lokal eine Implosion, vergleichbar der Kavitationswelle von Hochgeschwindigkeitsgeschossen. Die Energieumsetzung bewirkt einen ausgedehnten lokalen Gewebsschaden [6]. Die Fragestellung dieser Untersuchung war daher, ob und inwieweit ein unterschiedlicher Anstieg dieser vasoaktiven Mediatoren bei Elektiv eingriffen am Knochen (Osteotomie) im Vergleich zu Frakturen der unteren Extremität zu beobachten ist und ob diese Prostanoide hierbei als Maß der Verletzungsschwere dienen können.
