Zusammenfassung
Tumoren mit Ausdehnung im Bereich von Sella turcica und Clivus betreffen sowohl das neurochirurgische als auch HNO-ärztliche Fachgebiet. Tumoren dieser Region können sich nach intrakraniell als auch in Richtung Nasen-Rachen-Raum ausdehnen. Klassisches Beispiel hierfür sind die Clivuschordome. Sie können sowohl als weitgehend intrakranielle Tumoren in Erscheinung treten, bei ventraler Ausdehnung dagegen auch einen nasopharyngealen Tumor imitieren. Frontobasale Karzinome oder die von der Mukosa der Nn. olfactorii ausgehenden Ästhesioneuroblastome können Keilbeinhöhle, Sella oder Clivus erreichen. Andererseits weisen auch intrakranielle Tumoren gelegentlich ein invasives Wachstum in die Schädelbasis und bis in den Bereich der Nasen-Nebenhöhlen auf. Als Beispiel hierfür seien die Meningeome der Schädelbasis genannt. Abbildung 1 zeigt ein Hypophysenadenom, welches einen primären Tumor der Keilbeinhöhle und des Clivus vortäuscht. Die Sellaloge erscheint neuroradiologisch nicht betroffen, der Hypophysenkörper ist hier deutlich vom Tumor abzugrenzen. Genauso können sich raumfor-dernde Prozesse und vor allem Malignome des Naso-pharynx und der Nebenhöhlen nach intrakraniell ausdehnen. So bestehen bei Tumoren der Schädelbasis vielschichtige Überschneidungen der Fachrichtungen. Der HNO-Arzt Hirsch [23] schrieb 1957 über „Hypophysentumoren — ein Grenzgebiet“. Noch 1982 hat der Neurochirurg Derome [8] deswegen die Tumoren der Schädelbasis als „Niemandsland der verschiedenen Fachrichtungen“ bezeichnet. Heute basiert das Management von Tumoren der Schädelbasis vielmehr auf der Kooperation und dem Erfahrungsaustausch der beteiligten Fachrichtungen.
