Zusammenfassung
Dem transmuralen Myokardinfarkt liegt in 80- 90% der Fälle ein thrombotischer Verschluß des versorgenden Koronargefäßes zugrunde. Die Größe des Infarktes ist abhängig von der Verschlußlokalisation, dem koronaren Versorgungstyp sowie von einem präexistenten Kollateralkreislauf. Von wesentlicher Bedeutung darüber hinaus ist der allgemeine Koronargefäßstatus und der Funktionsgrad des nichtbetroffenen Myokards. Die Komplikationen des akuten und chronischen Krankheitsstadiums sind weitgehend abhängig von der sich entwickelnden Nekrosegröße und Infarktlokalisation. Mortalität sowie postinfarzielle Ventrikelfunktion werden ganz wesentlich von der Infarktgröße bestimmt. So ist die Infarktgröße wichtigster Parameter der klinischen Symptomatik, der verbliebenen Arbeitsleistungsfähigkeit und der Überlebenswahrscheinlichkeit.
