Zusammenfassung
Wenn heute von einer Therapie der Hypertonie gesprochen wird, ist vorzugsweise der Einsatz von Medikamenten gemeint. Medikamente stehen in großer Zahl zur Verfügung, sind wirksam, in zahlreichen prospektiven Studien erprobt, deswegen zur Langzeitkontrolle geeignet, teilweise billig (Thiazide, Betablocker) sowie vergleichsweise schnell und einfach zu verordnen. Sie müssen meist jedoch über Zeiträume von 1 bis 4 Jahrzehnte eingenommen werden und bergen damit alle Risiken und Kosten einer medikamentösen Langzeittherapie. Diese Überlegungen müßten eigentlich dazu führen, daß Arzt und Patient die Hypertonie hauptsächlich mit Allgemeinmaßnahmen (= natürliche, nichtmedikamentöse Heilmaßnahmen) zu behandeln versuchen, zumal Naturheilverfahren im weitesten Sinne eine hohe Akzeptanz bei der Bevölkerung haben, und die Wirksamkeit dieser Maßnahmen gerade bei der am meisten verbreiteten milden Hypertonie am besten erprobt ist. Dies ist jedoch nicht oder nur unzureichend der Fall, obwohl sich in neuesten prospektiven und kontrollierten Langzeitstudien (2, 4, 25, 36, 38) nachweisen ließ, daß eine effektive Blutdrucksenkung durch Allgemeinmaßnahmen erzielt werden konnte (Tabelle 1). Deswegen werden Allgemeinmaßnahmen als wirksame bevölkerungsweite Maßnahme zur Prävention der Hypertonie diskutiert (19). Allerdings wird sowohl in den neuesten Statement–Papieren zur Hochdrucktherapie von WHO/ISH (41) und JNC-V (19) und entsprechenden Kommentaren (40) auch darauf hingewiesen, daß eine Mortalitäts- oder Morbiditätssenkung durch Allgemeinmaßnahmen bisher nicht nachgewiesen wurde. Dieser Nachweis erfolgte jedoch kürzlich in der Stanford-Prevention-Studie (13).
