Die Klinik der Frühmobilisation des Herzinfarktpatienten

Bewegungstherapie in der Kardiologie

Zusammenfassung

Die Frühmobilisation ist als ein im frischen Infarktstadium beginnendes, am klinischen Bild der Erkrankung orientiertes Behandlungsprinzip mit Bewegungsreizen zu definieren. Das therapeutische Ziel liegt in der Prophylaxe aller primär nicht cardialen physischen und psy -chischen Komplikationen bzw. Funktionsminderungen, die durch eine Immobilisation pro -voziert werden können. Wie in den letzten 30 Jahren seit den ersten entscheidenden Impul -sen durch Levine und Lown (33), Haviar et al. (17) und Brummer et al. (10) in Verlaufskon -trollen (1, 2, 4, 8, 9, 13, 24, 29, 30, 34, 44, 46, 50, 51, 52) und in kontrollierten Studien (3, 7, 14, 15, 16, 19, 20, 31, 36, 48, 53) gezeigt wurde, führt dieses Vorgehen zu keiner Erhöhung der Frühletalität oder einem häufigeren Auftreten von schwerwiegenden akuten oder chronischen Komplikationen gegenüber dem lange Zeit geltenden Behandlungsdogma der mehrwöchigen strengen Bettruhe, das auf pathomorphologischen und nicht funktionellen Erkenntnissen basierte. Heute wird den Vorstellungen der WHO (54) von 1968 und denen des Rehabilitationsaus -Schusses der Internationalen Gesellschaft für Cardiologie (45) allgemein gefolgt, daß die Frühmobilisation die erste Phase der Rehabilitation des Infarktkranken darstellt. Über die klinischen Voraussetzungen, den Zeitpunkt des Beginns, über die Art der Mobilisationsmaßnahmen und den in der Klinik anzustrebenden Mobilisationsgrad bestehen jedoch unterschiedliche Auffassungen (18, 24, 43, 49), die sich in stationären Behandlungsdauern von 3 Tagen (1, 52) bis 3 Wochen (4, 7, 24, 51, 54) bei Patienten mit komplikationslosem Infarkt widerspiegeln.
