Zusammenfassung
Perkutane, transluminale Valvuloplastie (Valvulotomie) der Aorten-, Mitral-, Pulmonal- oder Trikuspi-dalklappe durch einen retrograden, antegraden oder transseptalen Zugang ist eine Alternative zur Herzklappenoperation von angeboren oder erworben stenosierten Herzklappen. Ballonkatheter unterschiedlichster Konstruktionsform werden mittels Führungsdrahts unter Röntgenkontrolle oder zusätzlicher Ultraschallkontrolle zwischen den eingeengten Klappensegeln positioniert und durch manuelle Insufflation zur Entfaltung gebracht. Das Ausmaß der Dehnung kann durch Wahl der Ballongröße und durch Steuerung des Ballondrucks variiert werden und richtet sich nach den zuvor invasiv und nichtinvasiv gemessenen Parametern sowie nach Erfahrungswerten.
