Zusammenfassung
An der Universitätsaugenklinik Tübingen werden seit den siebziger Jahren kombinierte Operationen (Keratoplastik und Kataraktextraktion) durchgeführt. In dieser Studie soll die Langzeiteffizienz dieser Triple-Operation überprüft werden.
Patienten: Untersucht wurden 23 Augen von 19 Patienten (Altersdurchschnitt 70 ± 8,4 Jahre), bei denen an der Universitätsaugenklinik Tübingen zwischen 1983 und 1991 eine Triple-Operation durchgeführt wurde. Es handelt sich ausschließlich um solche Patienten, deren Hornhauttransplantat mindestens 4 Jahre (6,5 ± 2,3) alt war.
Ergebnisse: Bei 20 Augen wurde die Keratoplastik erstmalig durchgeführt, bei 3 Patienten lag eine Rekeratoplastik vor. Der präoperative Visus besserte sich von durchschnittlich 0,06 ± 0,08 auf 0,4 ± 0,26. Der Interferenzvisus betrug postoperativ 0,5 ± 0,2. Intraoperative Komplikationen waren in vier Fällen eine vis-a-tergo-Reaktion und eine Vorderkammerblutung. Als häufigste postoperative Komplikationen waren ein Astigmatismus in 61% (3,4 ± 2,3 dpt) und ein Sekundärglaukom in 4,3%. 6 Patienten hatten in der postoperativen Phase zusätzliche Augenoperationen am betroffenen Auge. Schlußfolgerung: Unsere Ergebnisse zeigen, daß Triple-Operationen im Langzeitverlauf zu einer sehr stabilen Visusverbesserung führen. Deshalb können wir Triple-Operationen bei Patienten, die eine rasche visuelle Rehabilitation benötigen, empfehlen.
Summary
Since the 1970s, combined keratoplasty and cataract surgery has been performed at the University Eye Clinic in Tübingen. This study was carried out to evaluate the long-term efficacy of this procedure.
Methods: Only grafts that were followed for at least 4 years (6.5 ± 2.3) were analyzed. We studied the clinical outcome and final visual acuity of 19 patients (mean age 70 ± 8.4 years), who underwent triple procedere surgery in 23 eyes at the University Eye Clinic in Tübingen between 1983 and 1991. Results: In 20 eyes, keratoplasty was performed for the first time; three patients underwent re-grafting. The mean diameter of the donor transplant was 7.31 ± 0.4 mm compared to the host transplant of 7.17 ± 0.4 mm. Intraoperative complications included a vis a tergo reaction (n = 4) and one bleeding in the anterior chamber. The most frequent complications during the postoperative period were astigmatism (61%) and secondary glaucoma (4.3%). Six patients required additional surgical intervention. The preoperative visual acuity (0.06 ± 0.08) improved postoperatively to 0.4 ± 0.26 which was statistically significant. Postoperative retinal vision was 0.5 ± 0.2. Conclusion: Our results demonstrate the long-term success of the triple procedure, which can be especially recommended in patients with reduced general health and who need rapid visual rehabilitation.
