Zusammenfassung
Die thorakale Aorta bietet nicht nur in operationstechnischer Hinsicht Besonderheiten. Auch pathologisch-anatomisch unterscheidet sie sich bezüglich der Art und Häufigkeit dort auftretender Aneurysmen von der Bauchaorta und den übrigen großen Arterien. Dies geht deutlich aus einer sich auf 35 380 Autopsien beziehenden Erhebung über die Lokalisation verschiedener Aneurysmen hervor, wie sie 1980 von Kunz zusammengestellt wurde (15). Seine Daten belegen, daß atherosklerotische Aneurysmen ganz überwiegen (n = 854) in der Aorta abdominalis lokalisiert sind, während in der Aorta thoracalis nur 150 derartige Aneurysmen gefunden wurden. Demgegenüber wurden Medionekrosen bzw. dissezierende Aneurysmen in der Bauchaorta nur 15mal, in der Brustaorta dagegen 115mal festgestellt. An dieser Stelle soll zunächst offenbleiben, ob dissezierende Aneurysmen ätiologisch immer auf eine Medionecrosis aortae idiopathica Erdheim Gsell zurückzuführen sind, zumal viele dieser Fälle von einer mehr oder weniger ausgeprägten Atherosklerose begleitet werden.
