Minimal Invasive Surgery in Thoracic Outlet Syndrome

Summary

The thoracic outlet syndrome implicates a number of various diagnoses concerning either vascular and/or nerval compression syndromes. Best results are reported on transaxillary first rib resection in vascular compression. At our department, the diagnosis is based on a follow-up of clinical provocation tests. X-ray and intravenous digital subtraction angiography with positional provocation. The operation is only done on vascular compression using the transaxillary resection of the first rib. Eleven cases show good to excellent functional and aesthetic results, so that we rate this procedure to be an example of minimal invasive surgery.

Zusammenfassung

Das Thoracic Outlet Syndrom ist eine Zusammenfassung verschiedener Diagnosen, die entweder Gefäß- und/oder Nervenkompressionssyndrome beinhalten. Bei Gefäßkompressionen werden die besten operativen Ergebnisse mit der transaxillären Resektion der ersten Rippe erreicht. Nach klinischen Provokationstests, Röntgenbild und intravenöser digitaler Subtraktionsangiographie in Provokationsstellung führen wir bei vaskulärer Kompression die transaxilläre Resektion der ersten Rippe durch. Diese Vorgehensweise zeigt in 11 Fällen gute bis hervorragend funktionelle und ästhetische Ergebnisse, so daß wir dies als Beitrag zum schonenden Operieren erachten.
