Der kognitiv-verhaltenstherapeutische Ansatz und seine Anwendung

Zusammenfassung

Der kognitiv-verhaltenstherapeutische Ansatz bei chronischem Schmerz basiert auf den allgemeinen Prinzipien der kognitiven Verhaltenstherapie (KVT; Mahoney 1974; Meichenbaum 1977). Eine Grundannahme des Ansatzes ist es, daß Kognitionen entscheidend das Verhalten des Menschen mitbestimmen, d. h. daß Menschen ihr Handeln vornehmlich nach ihrer subjektiven Wahrnehmung der Welt und nicht nach objektiven Charakteristika der Ereignisse ausrichten. Des weiteren betont der kognitiv-verhaltenstherapeutische Ansatz die Bedeutung der Veränderung nicht nur des Verhaltens, sondern auch der damit einhergehenden Kognitionen (Überzeugungen, Einstellungen, Selbstinstruktionen) für die Aufrechterhaltung und Generalisierung einer therapeutischen Veränderung. Die Untersuchungen Turks (1975, 1977) zur Wirksamkeit von kognitiven Strategien bei Laborschmerz führten zur Entwicklung eines kognitiv-verhaltenstherapeutischen Therapieansatzes bei Patienten mit chronischen Schmerzen (vgl. Holzman et al. 1986; Turk u. Meichenbaum 1994; Turk et al. 1983). Die kognitiv-verhaltenstherapeutiche Intervention bei chronischen Schmerzen hat jedoch nur noch wenig mit den im Labor verwendeten Verfahren zu tun, da die Methoden an die Situation des chronischen Schmerzpatienten angepaßt wurden. Im Gegensatz zum operanten Ansatz wird neben dem Schmerzverhalten auch die subjektive Schmerzerfahrung explizit berücksichtigt. Ziel der Intervention ist jedoch nicht primär die Schmerzreduktion, sondern eine bessere Schmerzbewältigung, d. h. eine Verbesserung der Lebensqualität trotz der Schmerzen.
