Zusammenfassung

Bei einem Ausfall der 3 Schichten der Nebennierenrinde (NNR; Zona fasciculata, glomerulosa, reticularis) sistiert die Sekretion von Hydrocortison (Kortisol, 5,7 mg/qm/Tag) und Aldosteron (0,1 mg/ Tag) sowie der Androgene Dehydroepiandrosteronsulfat (DHEAS) und Androstendion (zusammen 30 mg/Tag). Da die Bildung von Hydrocortison und der NNR-Androgene vorwiegend von der hypophysären ACTH-Sekretion abhängt, die Aldosteronsekretion dagegen vorrangig vom Renin-AngiotensinSystem gesteuert wird, fallen bei der primären (d. h. adrenal bedingten) NNR-Insuffizienz alle 3 Hormonsysteme aus, bei einer sekundären (d. h. hypophysär bedingten) NNR-Insuffizienz ist die mineralokortikoide Funktion der NNR zumindest z. T. erhalten.
