Zusammenfassung
In das Konzept der multimodalen Therapie der Peritonealkarzinose gehört seit vielen Jahren die zytoreduktive Chirurgie (CRS) sowie die hypertherme intraperitoneale Chemoperfusion (HIPEC). Allerdings sind diese Verfahren nach wie vor mit einer nicht unerheblichen Morbidität und Mortalität verbunden. Da die wenigsten Patienten (mit Ausnahme des Pseudomyxoma peritonei) in kurativer Intention operiert werden können, steht die Patientenselektion im Vordergrund des präoperativen Managements. Das oberste Ziel muss es dabei sein, dass der Vorteil für den Patienten in der onkologischen Situation größer ist als das Risiko, dass er durch Komplikationen einen Nachteil im Sinne einer geminderten Lebensqualität erleidet. Scores zur Evaluierung der Paritonealkarzinose spielen daher in der Indikationsstellung eine zentrale Rolle.
