Assessment of Diastolic Function of Hypertrophic Heart by Doppler Echocardiography

Summary

Abnormalities of left ventricular diastrolic filling were studied in hypertensive patients and patients with hypertropic cardiomyopathy by the measurement of transmitral flow velocity patterns using pulsed Doppler echocardiography. Peak velocity in the early diastolic filling phase (peak E) and the ratio of peak velocity in the atrial contraction phase (peak A) to peak E were used as parameters of left ventricular diastolic filling. A decrease in peak E and an increase in the ratio of peak A to peak E were observed in hypertensive patients without apparent left ventricular hypertrophy, and a further decrease in peak E and a further increase in the ratio of peak A to peak E were observed in hypertensive patients with left ventricular hypertrophy. In patients with hypertrophic cardiomyopathy, a decrease in peak E and an increase in the ratio of peak E to peak A were observed only in patients with a decreased percentage of systolic wall thickening of the left ventricular posterior wall, and both parameters were normal in patients without a decrease in the percentage of systolic wall thickening of the left ventricular posterior wall. Thus, left ventricular diastolic filling in patients with hypertrophied hearts was noninvasively assessed with pulsed Doppler echocardiography.
